    <Abstract>Background: Type 2 diabetes mellitus (T2DM) is one of the most prevalent metabolic disorders worldwide and is characterized by impaired glycemic control and dyslipidemia, both of which substantially increase the risk of cardiovascular disease. Resistance training and herbal supplements such as garlic (Allium sativum), particularly its bioactive compound allicin, have each demonstrated beneficial effects on glycemic regulation and lipid metabolism. However, the potential synergistic effects of combining these interventions have received limited attention.
Objective: This study was designed to investigate the effects of 10 weeks of resistance training combined with garlic (allicin) supplementation on the lipid profile, including total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), triglycerides (TG), and fasting blood glucose (FBG) in middle-aged men with type 2 diabetes mellitus.
Methods: In this randomized controlled clinical trial, 48 middle-aged men (45–60 years) with type 2 diabetes mellitus were randomly assigned to one of four equal groups (n = 12 per group): (1) resistance training plus garlic supplementation, (2) resistance training plus placebo, (3) garlic supplementation alone, and (4) control (placebo without exercise). Participants in the training groups performed a progressive resistance training program three times per week for 10 weeks. Participants assigned to the supplementation groups received 600 mg/day of standardized garlic powder, equivalent to 3.6 mg of allicin, throughout the intervention period. Fasting venous blood samples were collected at baseline and after the 10-week intervention to determine serum concentrations of total cholesterol, LDL-C, HDL-C, triglycerides, and fasting blood glucose.
Results: After 10 weeks, the resistance training plus garlic supplementation group demonstrated significant reductions in fasting blood glucose (P &lt; 0.001), total cholesterol (P &lt; 0.001), LDL-C (P &lt; 0.001), and triglycerides (P = 0.01), along with a significant increase in HDL-C (P &lt; 0.001) compared with the control group. Furthermore, significant synergistic effects were observed between resistance training and garlic supplementation for improvements in fasting blood glucose (P = 0.01) and total cholesterol (P = 0.05).
Conclusion: Ten weeks of resistance training combined with garlic (allicin) supplementation produced synergistic beneficial effects on glycemic control and lipid profile in middle-aged men with type 2 diabetes mellitus. These findings suggest that this combined intervention may represent an effective non-pharmacological strategy for the management of patients with type 2 diabetes mellitus.</Abstract>
    <OtherAbstract Language="FA">مقدمه: دیابت نوع ۲ یکی از شایع ترین بیماری های متابولیک در جهان است که با اختلالات گلیسمی و دیس لیپیدی همراه بوده و خطر بیماری های قلبی-عروقی را به طور قابل توجهی افزایش می دهد. تمرین مقاومتی و مکمل های گیاهی مانند سیر (آلیسین) هر یک به تنهایی اثرات مفیدی بر کنترل گلیسمی و پروفایل لیپیدی نشان داده اند، اما اثر هم افزایی آن ها کمتر مورد بررسی قرار گرفته است.
هدف: با هدف بررسی تأثیر ۱۰ هفته تمرین مقاومتی همراه با مصرف مکمل سیر (آلیسین) بر پروفایل لیپیدی (کلسترول تام، LDL، HDL، تری گلیسرید) و قند خون ناشتا در مردان میانسال مبتلا به دیابت نوع ۲ طراحی شد.
روش ها: در این پژوهش کارآزمایی بالینی تصادفی سازی شده با گروه کنترل، ۴۸ مرد میانسال (۴۵-۶۰ سال) مبتلا به دیابت نوع ۲ به طور تصادفی به چهار گروه مساوی(n = 12) شامل (۱) گروه تمرین مقاومتی + مکمل سیر، (۲) گروه تمرین مقاومتی + دارونما، (۳) گروه مکمل سیر (بدون تمرین)، و (۴) گروه کنترل (دارونما بدون تمرین) تقسیم شدند. گروه های تمرینی به مدت ۱۰ هفته (۳ جلسه در هفته) برنامه تمرین مقاومتی پیشرونده را اجرا کردند. گروه های دریافت کننده مکمل، روزانه ۶۰۰ میلی گرم پودر سیر استاندارد شده (معادل ۳/۶ میلی گرم آلیسین) دریافت نمودند. نمونه های خون ناشتا در ابتدا و پس از ۱۰ هفته برای اندازه گیری کلسترول تام، LDL، HDL، تری گلیسرید و قند خون ناشتا شد.
یافته ها: پس از ۱۰ هفته، گروه تمرین مقاومتی + مکمل سیر کاهش معناداری در قند خون ناشتا (P &lt; 0.001) کلسترول تام(P &lt; 0.001)، همچنینLDL (P &lt; 0.001) و تری گلیسرید(P = 0.01) و افزایش معناداری در(P &lt; 0.001) HDL نسبت به گروه کنترل نشان داد. اثر هم افزایی معناداری بین تمرین مقاومتی و مکمل سیر در بهبود قند خون ناشتا(P = 0.01) و کلسترول تام(P = 0.05) مشاهده شد.
نتیجه گیری: ۱۰ هفته تمرین مقاومتی همراه با مصرف مکمل سیر (آلیسین) اثر هم افزایی بر بهبود کنترل گلیسمی و پروفایل لیپیدی در مردان میانسال مبتلا به دیابت نوع ۲ دارد و می تواند به عنوان یک راهکار غیردارویی مؤثر در مدیریت این بیماران توصیه شود.</OtherAbstract>
